School Enrollment
Welcome to St. Joseph Academy! We’re glad you’ve decided to learn more about our school and look forward to working with you and your child as members of our school family.
Enrollment applications for the 2021-2022 school year opened on February 1, 2021. School enrollment occurs every month between the 1st and the 20th – though sometimes it may be necessary to put your child on a waiting list if our classes are full.

Documents Needed
–If you are a NEW student (not currently enrolled), we will need the following for each child:
• Birth certificate (with raised seal) OR immunization record
• Income verification (any of the following)
– WE Energies bill
– Paycheck stubs with address of a parent on the application
– Any government letter
– Cable bill (not internet unless you bundle)
– Sewer or water bill
– Current lease
– W-2’s
– All documents must be the recent bill/letter from time of application.
– All documents must have the name of the parent on the application.
• Proof of income
– 2020 federal income tax for 1040 (2 pages)
If you have not filed your taxes by the time of the application, 2020 W-2’s for all parents/guardians listed on the application are acceptable if you apply before April.
– 2020 1099 form for any Social Security benefit received
If you are a RETURNING student (currently enrolled), your only required to submit proof of residency.
Medical Forms
Upon acceptance into St. Joseph Academy, you will be asked to complete forms that allow us to understand your child’s medical history, specifically if they have medical conditions, food allergies, special dietary neeeds, and prescriptions that need to be administered.

EARLY CHILDHOOD EDUCATION CENTER (ECEC)
St. Joseph Academy provides your infant, toddler or preschooler with a loving, nurturing environment that supports them physically, educationally and emotionally. We are committed to working with you and providing the best care possible for your child.
SJA’s ECEC program provides care for children 6 weeks through 4 years and also had Early Head Start and Head Start classrooms. Please contact Brenda Rodriguez at brodriguez@sjamilwaukee.org to see if you meet income and other eligibility requirements for Early Head Start or Head Start.
To begin you child’s application, please fill out the ECEC Interest Form and either mail in or bring the form to the main office.
Eligible families may be able to have a portion of their Early Childcare program costs covered by the Wisconsin Works (W-2) program. Contact our Enrollment Coordinator, Brenda Rodriguez at brodriguez@sjamilwaukee.org or 414-645-5337, ext. 267.
